 Please assist us by providing the following information when you call and at the time of your first appointment:
- Your referral slip and any X-rays if applicable
- A list of medications you are presently taking
- If you have dental insurance, please have your card available. This will save time and allow us to help you process any claims.
IMPORTANT: All patients under 18 must be accompanied by a parent or guardian during all visits.
Please alert the office if you have a medical condition that may be of concern (i.e. latex allergy, history of hip or joint replacement or allergies to any medications).
